Jellyroll Enrolment Form

If you would like to enrol your child at Jellyroll, the usual way is to return the completed form and deposit to Joanne Smith at the address below. (An internet application form will be held pending until deposit has been received) Alternatively, children are welcome to come for a trial session on a prearranged Saturday (£14 infants £24 seniors) before making a commitment. Should they wish to continue with us, we then invoice from the date they start. However, we wouldn't be able to guarantee places on this basis Should a student wish to leave, one half term's notice will be required in writing by midterm. Click Appropriate Button	A) JELLYCATS (9yrs and over) 9.30am-12.30pm £240.00 full morning class per term, payable one week prior to commencement of the term. A deposit of £40 is required to register and secure a place at Jellyroll, this will be deducted from the first term's invoice.

	C) JELLYTOTS (4yrs-6yrs) 9.30am-11am Fees are £120 per term, payable one week prior to commencement of the term. A deposit of £20 is required to register and secure a place at Jellyroll, this will be deducted from the first term's invoice.
